Why do big companys make horrible designs?

like the ipod or a cd case or phone or backpack mainly and to me the most inportant one is cig-er-et (im on the ipod so hard to spell right with the bad design) main flaw for those are they cause lung cancer why dont companys or industrys take time designing and testing and sorry if annoyed people who smoke with this question . if you answerd this question thanks
